HOW IT WORKS
SIGN UP & SUBMIT PAYMENT
Choose one of three options:
1) Dedicate your full LEF ($500) to IOME services to receive five, 50-minute sessions.
2) LEF pays for your IOME membership and you choose how much of your LEF you'd like to dedicate towards IOME (receive services as requested/needed).
3) Pay for session upfront without membership and submit reimbursement form to Larimer County and receive payment back minus the $25 copay.

REPORT YOUR PAYMENT TO THE COUNTY
1) Keep your IOME receipts and submit to your HR department for reimbursement.
2) LEF pays for your IOME membership and you receive a superbill to submit for reimbursement each time you receive a service.
